All students are required to complete and submit health forms and immunization documentation before they can register for classes at Adelphi.

Medical Requirements and Immunizations for Adelphi Students

As a new student, you must meet New York State immunization requirements and the Health Services Center standards:

  1. A completed Health History and Assessment form.
  2. Proof of immunization against measles, mumps and rubella on the Immunization Requirements form.
  3. A completed Meningococcal Meningitis Vaccination response form.

These forms are sent out with the accepted student’s packet and can be downloaded from the Health Services Center website or picked up from the health Services Center in Waldo hall. The forms must be completed and signed by your healthcare provider and returned prior to registration.

  • Student Teaching Clearance
  • Nursing Student Clearances: College of Nursing and Public Health (CNPH) students will have additional health clearance requirements that must be completed prior to beginning their upper division nursing courses and clinical practicums. The CNPH Clinical Placement office will notify all students of the requirements and deadlines via email. For any nursing health clearance questions, you may contact clinicalplacement@adelphi.edu.

Adelphi needs proof of required immunization for medical clearance. If you do not comply with the New York State mandated immunization standards. You will not be allowed to attend class and may be withdrawn from the University.

Students enrolled in fully online programs are not required to provide immunization records. If at any time this status changes and the student begins on-site classes, they will be required to provide proof of all necessary immunization requirements.
