Daily Health Monitoring Requirements
As we return to campus, effective immediately, the University requires all Adelphi students, faculty and staff to conduct a COVID-19 health screening every day before entering campus.
Long Island entered into Phase Four of New York Forward in early July, the final phase of reopening New York which permits the re-opening of higher education for teaching, learning, and student life. Abiding by state guidelines, Adelphi plans to reopen for in-person learning this Fall 2020.
Adelphi is committed to safeguarding the members of our community and helping to prevent further spread of the coronavirus to our communities.
In order to enter campus, you must be free of symptoms potentially related to COVID-19, or have been medically cleared by a physician (staff and faculty) or Student Health Services (students). Student Health Services can be reached at firstname.lastname@example.org or 516.877.6000.
For daily health monitoring, the COVID-19 health screening questionnaire is accessible via the AU2GO mobile app. If you haven’t already, please download the AU2GO app and self-screen before arriving on-campus.
If you do not have access to a device, you can also complete the COVID-19 health screening questionnaire via eCampus on a desktop at home and print the result screen, or fill out a paper form at the Public Safety booth on South Avenue or other designated campus entrance, before entering campus.
Your Health Monitoring Checklist
According to the CDC guidance on “Symptoms of Coronavirus,” people with COVID-19 have had a wide range of symptoms reported, ranging from mild symptoms to severe illness. For the safely and health of all our community members, please answer every question honestly, even if you feel you have a cold, allergies or explainable muscle aches.
Our Office Student Health Services is available and ready to provide guidance, direction and support if you do not feel well, however, in order to enter campus, you must be free of symptoms potentially related to COVID-19, or have been medically cleared by a physician (staff and faculty) or Student Health Services (students). - In the past 10 days, have you tested positive for COVID-19?
- In the past 10 days, have you experienced any of the following possible symptoms of COVID-19?
- Shortness of breath or difficulty breathing
- Fever over 100.4 degrees
- Muscle pain
- Sore throat
- New loss of taste or smell
- Rash on hands or feet
- Pain or redness in toes
- In the past 10 days, have you knowingly been in close contact with anyone who has tested positive for COVID-19 or who has or had symptoms of COVID-19?
- Have you traveled to a restricted state in the past 10 days? If so, have you quarantined for 10 days upon entry to NY?
- Do you have a face covering or mask (or agree to pick one up from a designated Campus location immediately upon arrival), and do you agree to wear It while on Adelphi property?
Acceptable face coverings for COVID-19 include but are not limited to cloth-based face coverings and disposable masks that cover both the mouth and nose; bandanas, buffs, and gaiters are not acceptable face coverings, and masks with valves or vents do not offer adequate protection and should not be used while on campus.
- Do you agree to abide by CDC Social Distancing guidelines while on Adelphi property?
A distance of at least six feet is to be maintained among individuals at all times, unless safety of the core activity requires a shorter distance. Any time individuals must come within six feet of another person, acceptable face coverings must be worn. Individuals must be prepared to immediately wear a face covering if another person unexpectedly comes within six feet.
If you are experiencing any of the above COVID-19 symptoms, please remain home. If you are experiencing any of these symptoms while at work, please leave campus immediately and contact your healthcare provider for assessment and testing.
