Concern About Worrisome Behavior

Use this form to report student or employee behavior you are concerned about, and which might present a risk to themselves or othersIF THE BEHAVIOR PRESENTS AN IMMINENT THREAT, PLEASE CALL PUBLIC SAFETY AT 516.877.3500 OR DIAL 911 right away. This report will go to the interdisciplinary Behavioral Intervention Team and will be read within one business day. (Students or employees have a right to view the report if they request to, but it will not show  your personally identifiable information.)

Concern About a Student’s Wellbeing

Use this form to report non-urgent but concerning student behavior or sense of wellbeing. It will be sent to the Division of Student Affairs.

Follow-up Incident Report

The follow-up incident report is used to provide additional information about a previously reported incident. The follow-up incident report can be used to provide information about an incident reported to any department that uses the Maxient Conduct Manager software to receive documentation about incidents, including:

  • Student Conduct and Community Standards
  • Title IX and Sexual Respect
  • CARE Team
  • Behavior Intervention Team
  • Community Concerns and Resolution
  • Housing and Residence Life
