Use this form to report incidents of academic misconduct. Academic misconduct includes, but is not limited to:
- Use of unauthorized sources
- Prohibited behaviors noted on a class syllabus
Concern About Worrisome Behavior
Use this form to report student or employee behavior you are concerned about, and which might present a risk to themselves or others. IF THE BEHAVIOR PRESENTS AN IMMINENT THREAT, PLEASE CALL PUBLIC SAFETY AT 516.877.3500 OR DIAL 911 right away. This report will go to the interdisciplinary Behavioral Intervention Team and will be read within one business day. (Students or employees have a right to view the report if they request to, but it will not show your personally identifiable information.)
Concern About a Student’s Wellbeing
Use this form to report non-urgent but concerning student behavior or sense of wellbeing. It will be sent to the Division of Student Affairs.
Use this form to report incidents of sexual misconduct and behavior that violate Title IX.
- Dating and domestic violence
- Gender-based discrimination
- Sexual harassment
- Sexual assault
Use this form to report general incidents of misconduct. Incidents include, but are not limited to:
- Alcohol use
- Drug use
- Housing policy violations
- Physical assault
- Organizational violations
- Hazing incidents
- Disruptive behavior

Use this form to submit an appeal of your student conduct decision.
