Photo/Video Release Form
If you are taking a picture, or shooting a video, where individuals are clearly identifiable, each individual featured must sign the form below. Please keep a copy of the form for your records. Marketing and Creative Services will not use photos if people featured in them do not have a release formed signed and on record.
